ACCORD Corporation’s Head Start Division is accepting quotes for a Health Care Consultant for the period of July 1, 2020 – June 30, 2025. Our program currently serves 260 children ages birth to five years.
Documents Required:
1. Certification: Must be certified as Registered Nurse. Must provide a current resume and copy of registration and degree in Health.
2. A certificate of liability insurance must be provided.
Requirements and Responsibilities:
· Knowledge of and experience implementing Office of Children and Family Services Health Care Consultant responsibilities including:
o Initial visit to each center to create or review the Health Care Plan (HCP) and ensure proper implementation of such plan.
o Complete a follow up Health Care Plan compliance site visit during first year of consultant services.
o Complete the two year OCFS required site visits to review Health Care Plan (HCP) and related practices and certifications including:
§ a random sampling of: immunization records, daily health checks, medical forms and blue cards for children in care,
§ a review of Individual Health Care Plans for Children with Special Health Care Needs,
§ a review of MAT certificates and CPR/FA certifications for MAT staff,
§ observation of first aid kit contents for use in childcare setting,
§ review of medication consent forms and medication documentation logs, and
§ medication administration procedure and medication storage practices.
o Additional visits or technical assistance by phone upon request for issues or practices relating to the health care plan, staff training, etc.
